The Primary
Health Network (PHN) was founded in 1984, with one small site. Our goal
has always been to provide quality medical care to the people we serve. Since
our inception, PHN, a private, nonprofit organization, has worked with local
communities to implement a new model of healthcare: the Community Health
Center.
The Primary
Health Network (PHN) began as one small community health center, since growing
to become the largest Federally Qualified Health Center (FQHC) in Pennsylvania,
with over 45 sites in 16 counties throughout Pennsylvania and Ohio. We employ a
medical staff that consists of 165+ physicians, dentists, physician assistants,
certified nurse practitioners, and other health professionals. Our support
staff includes over 600 employees, as well. Each year, we reach over 118,000
patients through health and educational services.
We offer
comprehensive primary care and numerous specialty services, as well as working
within communities by participating in health fairs and networking with local
organizations. Our management is structured to engage and interact with
communities, as the Board of Directors is composed of volunteer community
leaders and residents from throughout PHN’s service area. These members guide
PHN, encouraging community involvement.
At PHN, we
strive to assist our patients toward a fuller, healthier lifestyle. As part of
this drive, we offer health education, free pregnancy tests and blood pressure
screens at medical sites, and patient transportation in select counties
(Beaver, Clarion, Erie, Lawrence, and Mercer) to medical appointments. Personal
attention, convenience, and expert care are available throughout the network,
where you will feel at home through our patient-centered approach. The
Electronic Health Record (EHR) has been implemented throughout the network,
allowing PHN to keep up with technological advances in healthcare delivery.
Adapting to the changing healthcare market, PHN participates in most managed
care plans, processes most insurances, and offers a Sliding Fee Scale for
discounts to eligible patients.
The Primary
Health Network, a Federally Qualified Health Center, provides quality primary
care services and access to specialty care commensurate with the needs of the
people in the communities we serve. Services are offered regardless of age,
race, creed, sex, national origin or ability to pay.
